Collaborative care is a team-based behavioral health support model that puts the patient at the center. The referring provider connects patients to Triplemoon's behavioral health coaches who provide highly personalized and evidence-based behavioral health support via telehealth conferencing.
The primary care provider continues to provide care for physical health medical needs and expands their abilities to manage behavioral health. Collaborative care is based on a core set of principles that use consistent measurement to target outcomes alongside evidence based treatments.
By working in partnership with the primary care provider the overall patient experience and outcomes are improved significantly alongside the experience of the provider.

Triplemoon coaches are highly trained experts with an average of 5+ years of experience in a clinical setting (nursing, social work, etc) working specifically with pediatric populations. All coaches undergo rigorous behavioral health training at onset and didactic instruction utilizing Triplemoon's proprietary evidence-based curriculum to ensure we bring the leading research to our patients.

There are many differences. Most importantly Collaborative Care has the largest evidence base for improved outcomes and is created to be implemented specifically in a primary care setting. More than 90 randomized controlled trials support collaborative care effectiveness in treating behavioral health conditions across a broad range of populations. Specifically, collaborative care has been shown to drive improved outcomes for both patients and doctors with most patients seeing a 50% improvement in behavioral health symptoms after only 12 months of treatment.
Traditional therapy focuses on addressing both past and present topics and has a "longer term" horizon. Families often struggle to find a therapist that is "in-network," costs to the family are high, and wait times can exceed 3+ months. During this time, primary care providers lose transparency into their patient's experiences and outcomes.
In contrast, Collaborative Care is well suited to address forward-looking behavioral health issues and can be provided almost immediately for patients at the referral of the primary care provider. Triplemoon's collaborative care model is evidence-based and measurement-oriented. We focus on identifying the "root issue,” creating a core set of goals and then working in a highly personalized manner to implement change.
Our relationship with each patient typically lasts under 12 months and visits range from 30 minutes to 60 minutes (or can be as short as 15 minutes)—100% personalized to each patient's needs. At each interaction the Triplemoon care team uses a combination of evidence based measurements and interventions to support patient behavioral health improvement. Throughout treatment the primary care provider remains part of the care team with regular patient progress updates provided to the practice. This team-based behavioral health model improves not only the quality of care for patients but also the quality of life for providers.
